Office Policy on Managed Care Insurers

McCalla Office2
In order to accommodate the needs and requests of our patients, we have enrolled in several managed care insurance programs. While we are pleased to be able to provide this service to you, it is extremely difficult for us to keep track of all the individual requirements of the different plans. Providing quality care for our patients is our primary concern. With your cooperation and help this can be accomplished with success.

Financial Policy

The patient or their guarantor is responsible for payment in full for services provided by McCalla Ear, Nose and Throat, P.C. at the time of service. We accept cash, checks, MasterCard, Visa, Discover. There is a $35.00 charge for returned checks.

The only exception for payment in full is if McCalla Ear, Nose and Throat, P.C. has contracted with your HMO/PPO/POS to accept the insurance payment after all deductibles have been met and all co-pays have been paid.

If you have insurance coverage through a company that we have contracted with, we require a copy of your insurance card, mailing address, and payment of your deductible and / or co-payment at the time of service. Failure to provide this may require you to pay in full at the time of service.

Surgery Fees

All surgeries arc pre-certified with your insurance company prior to the date of surgery. This can sometimes cause a delay in scheduling due to your plan's requirements. On some elective procedures, such as cosmetic Rhinoplasty (nasal plastic surgery), payment in full is required prior to the surgery. Some procedures may be considered cosmetic and are not covered by any insurance policy. We are willing to consider a payment plan.
